Open the menu and tap Restore purchases, or tap Restore purchases on the unlock card. Use the same Apple ID or Google account you bought it with. Unlock everything is a one-time purchase, so it comes back on a new phone at no charge.

If the app says no purchase was found, check that you are signed in to the right account in your phone's store settings, then try again.
