Open Hearth Pass in the app and tap Restore purchases. Use the same Apple ID or Google account you bought it with. Hearth Pass is a one-time purchase, so it comes back on a new phone at no charge.

If the app says no purchase was found, check that you are signed in to the right account in your phone's store settings, then try again.
